You can place your order for medical ID jewellery in one of several ways:
Order Online: Choose the item you wish to buy. After making this selection, you will then be asked to provide instructions for engraving the ID. Finally, you will be asked for payment and delivery information. Online orders can be paid with a credit card.
Order by Fax: If you prefer, you can print the order form here. Select the correct order form for the item you wish to purchase. Next, complete the item selection box, engraving information, and payment and delivery information. Payment may be made with a credit card. Fax your order directly to us at 0800 066 4485.
Order by Post: Print the order form here. Select the correct order form for the item you wish to purchase. Fill out the form completely and send it to us along with engraving, payment and delivery information. Payment can be made with a cheque, postal order or credit card. Post your completed form to:

Most orders will be dispatched within two weeks. Urgent shipping available at additional cost.
Please note that all offers to purchase goods are only valid upon acceptance of same by the company at its head offices in Houston, TX. Upon acceptance of same, a legally binding sale of goods agreement shall exist. Sales of goods by the company to customers shall be on an ex warehouse basis.
